Eligibility Requirements

Overview

Oak Point University motivates all students to nurture self-health by complying with University health requirements and any additional and site-specific requirements delegated by the school’s clinical partners. Health requirements applicable to all programs offered by Oak Point include immunizations, annual requirements, and conditional requirements. Compliance is contingent on the successful completion of conditional requirements. Some programs have program-specific requirements that must be completed. All students are expected to abide by the University’s compliance policy throughout program enrollment.

Castlebranch

Oak Point University partners with CastleBranch to track student health compliance and ensure student success. The system allows students to order their background checks and drug tests and to store all required documents throughout the duration of their program.

Your enrollment fee includes the initial cost of your CastleBranch account, with any ongoing clinical site-specific requirements requiring additional fees. The exception to this is students who defer enrollment and require a repeat drug test and/or background check. Repeated requirements require the student to pay the associated fees.

Clinical Sites

Please note that many of our clinical partners have additional requirements. Students are expected to complete clinical site compliance requirements in a timely fashion and by the deadline set by the Clinical Placement Office. Some sites require more recent health requirements, including but not limited to drug tests, TB/PPD, background checks, or fingerprints. Students are responsible for any additional costs associated with clinical.

Immunizations

Hepatitis B

Step 1: Complete a Hepatitis B Surface Antibody (Hep BsAB) with lab report—only titer accepted

  • If positive, no further action required—student is immune
  • If negative, a subsequent requirement is needed:
    • If a student has proof of a negative titer with the original series, they can:
      • Submit proof of negative titer and original series
      • A new requirement will be created for student to receive the Hepatitis B booster
    • If a student has proof of a negative titer without the original series, they have to:
      • Repeat the series of three (3) Hepatitis B vaccines
      • Provide a repeat titer two (2) months after the third vaccine

Student’s name must be displayed on documentation.

Measles (Rubeola)

Step 1: Submit an antibody (IgG) titer (lab report required) for Measles (Rubeola).

  • If positive, no further action required—student is immune
  • If any titer is negative or equivocal, a new requirement will be created for student to receive one booster shot.
Mumps

Step 1: Submit an antibody (IgG) titer (lab report required) for Mumps.

  • If positive, no further action required—student is immune.
  • If any titer is negative or equivocal, a new requirement will be created for student to receive one booster shot.
Rubella

Step 1: Submit an antibody (IgG) titer (lab report required) for Rubella.

  • If positive, no further action required—student is immune
  • If any titer is negative or equivocal, a new requirement will be created for student to receive one booster shot.
Varicella (Chicken Pox)

Step 1: Submit an antibody (IgG) titer (lab report required) for Varicella (Chicken Pox).

  • If positive, no further action required—student is immune
  • If any titer is negative or equivocal, a new requirement will be created for student:
    • First vaccine administered
    • Second vaccine administered 6-8 weeks after first vaccine
Tetanus, Diphtheria + Pertussis (Tdap)
  1. Documentation of a Tdap vaccine must be within the past 10 years.
  2. Tdap is the only vaccine accepted for this requirement.
  3. Not accepted: Dtap, TD, DT

Annual Requirements

CPR Certification

American Heart Association Basic Life Support for Providers course—this is the only course that is accepted for this requirement.

  1. Course may be taken online
  2. Skills portions MUST be taken via an in-person class

Acceptable documentation is:

  • Hard copy of signed card (both front and back)
  • Ecard
Tuberculosis (TB)

Step 1: Submit one of the following for Tuberculosis (TB):

  • One series of 2 Step PPD skin test (initial placement with reading 48-72 hours after placement)

OR

  • Blood test: QuantiFERON Gold OR T-Spot

If negative, submit documentation for approval.

  • Renewal will be set for one year from test date on the first day of the month the test will expire

If positive, a subsequent requirement is needed:

  • Positive Signs and Symptoms form signed by healthcare provider
  • Chest X-ray with determination of no active tuberculosis within five years
  • Renewal will be set for one year from test date on the first day of the month the test will expire
Influenza (Flu)

Submit documentation of an influenza (flu) vaccine for the current flu season (summer semester start students must have vaccine for previous flu season).

Documentation must include the following information:

  • Student Name and Birth date
  • Vaccine Manufacturer
  • Lot Number
  • Vaccine Expiration Date
  • Date given

The renewal date will be set for 10/01 the following flu season.

Health Insurance

All students are required to have and maintain continuous health insurance coverage throughout their time in the program. If a student’s health insurance expires or changes, students need to submit proof of updated renewal.

  • Students must submit a copy of the front of their insurance card
    • During the duration of the program, it is the responsibility of the student to update health insurance information in CastleBranch as needed
  • If student name does not appear on the card, in addition to the front of the insurance card, student must provide the following documentation from the insurance carrier:
    • Current Date
    • Name of person covered on policy
    • Policy number
    • Student name listed as dependent

Oak Point does not offer health insurance nor affiliate with any health insurance companies.

Conditional Requirements

Drug Test

Once student account is set up:

  1. CastleBranch will send each student an email with a registration form and instructions on how to access a list of approved lab sites student can go to for their urine test.
  2. If the email is not received with 24–48 hours of setting up the account, student should immediately call CastleBranch for additional information (888.723.4263).
  3. Do not wait to go in for the urine test. While most drug tests clear within a week, occasionally it can take up to 30 days for one to clear.
  4. Student MUST bring the registration form with them to the lab.
  5. If student’s drug test comes back flagged for drugs, student will be asked by CastleBranch to provide a note from their healthcare provider indicating the prescription they are taking to cause the positive results.
  6. If student cannot show proof of a prescription, student will be contacted by the Office of Admissions regarding their acceptance.
Background Check

During the ordering process, students will be asked to download an “Illinois Statewide Criminal Release” form that must be signed and returned within five days of setting up their CastleBranch account in order for the background check to be processed.

  1. Background checks which come back flagged for a conviction must be reviewed by HR.
  2. A felony conviction will prohibit any student from starting their program.
